[ARCHIVED] Parent/Caregiver Survey

The purpose of this survey is to learn what parents/guardians/caregivers think about some of the more challenging issues related to raising children and teenagers.

At the end of the survey, you can enter a drawing for a $100 Visa gift card!  If you'd like to enter the drawing, we'll ask you to fill out another form that's not connected to your survey answers.  The link to the form will be provided at the end of this survey.  Although we'll have your name and contact information, we won't be able to link your answers in this survey with you personally.   
 
To be eligible for a gift card, you must have a mailing address that falls within Hampshire, Hampden, or Worcester Counties. This is to prevent survey fraud. Please note that if selected, the gift card will be mailed to your address. We will not be providing the gift card via email or electronically. 
 
We encourage you to share the survey with the other caregivers in your life! Multiple members of a household - for example, a mother, a father, a grandfather, an aunt, etc. - are eligible to take it, given that they are a parent, guardian, or regular caregiver of a child in the survey region. 
 
The survey topics include attitudes about alcohol and drug use among young people, and how expectations are typically communicated at home and by other adults in the community. 

For the purposes of this questionnaire, we use the term "teenager" broadly to include both middle and high school youth. If you have more than one child in middle or high school, please think about your oldest child and his/her/their peers as you answer the questions.  

We recognize that people have a variety of beliefs on what is “normal” teenage behavior, and we’re truly interested in finding out what your beliefs are. The survey is voluntary and confidential. If there are any questions you don't understand or would prefer not to answer, feel free to skip. 
 
In the coming months, we will be using survey results to help develop strategies to communicate positive messages that reinforce healthy decision-making among students and support communication in families.
