Report My Meal

The Department of Public Health (DPH) is excited to share that a new, online system for reporting suspected foodborne illness to state and local health officials is now live: https://www.mass.gov/info-details/report-my-meal

Information entered in the new system, called Report My Meal, generates a foodborne illness complaint (FBI) event in MAVEN. The form can be completed by members of the public or health department staff who take in complaints from the public. You do not need to be a MAVEN user to submit complaints to Report My Meal. This can streamline complaint intake by facilitating what information should be collected without having to log into a system. Once the report is submitted, the information generates an FBI complaint event in MAVEN. If you are a MAVEN user who submits a complaint through Report My Meal, you can log into MAVEN and add information, notes, or attachments into the event after it is created.

Local board of health (LBOH) MAVEN users have immediate access to FBI complaints with their jurisdiction listed as where an ill individual lives or where a listed food establishment is located. New complaints associated with your jurisdiction can be found in the “LBOH Foodborne Illness (FBI) Complaint Notification” workflow. 

When an FBI complaint is received from Report My Meal, the “Complaint Source” field in the Administrative question package in MAVEN will be populated with “Online public complaint.”

The form is currently live making it accessible to the public and all LBOHs. DPH will be promoting Report My Meal on its social media pages in the coming months. Feel free to share this tool further with members of your community.
